Error in port widths or dimensions while interfacing the Joystick block with Simulink blocks

Dear all,
I am currently using the 3D joystick block in Matlab Simulink to interface my computer with the Logitech G25 Wheel using the MOMO_INPUT_signal.mdl.
When I am using the simple sinuoidal input block directly as a force-feedback input, the system runs perfectly and I can see the steering wheel
moving in either direction. But when, I interface this model with a state-feedback controller for steering angle control as shown below, I get an error message stating that
Error in port widths or dimensions. Input port 1 of 'steering_input_signal_closed_5/Outputs_Axes_POV' has 8 elements. This port does not accept
the dimensions (or orientation) specified by the input signal.
I tried my best in resolving this issue, but the problem still persists. I can see that there is a question mark on the top adder block getting the
input from In1yawd.mat which is quite strange. I have cross-verified all the gain matrices and found out that the matrix multiplication option
is provided in all the blocks It is my humble request to all of you to kindly assist me to sort out this issue.
Please find the snapshots of my simulink files attached along with this email. I am also sending you the simulink model files and the vehicle data file for your reference.
