Is this formula correct for the LME model I'm trying to fit?

I'm trying to fit a linear mixed effects model to investigate how time (fixed effect), disease duration at baseline, sex and age at baseline (all random effects) influence "power". I would like the intercept and slope to be allowed to vary per patient, and account for how sex and age might influence the effect of time on power. Would this formula address this?:
Power=Time+(1|Disease duration at baseline)+(1|Sex)+(1|Age at baseline)+
(Time|Patient) +(-1+Time since T1|Sex)+(-1+Time|Age)
