How to denoise this image?

I have two image
  1. Noisy Image
2. Targetted Image
I am totally confused and tried different filters like Weiner, median, bilat, etc but confused about how to proceed for this to denoise this image?
figure;
subplot(2,2,1);
imshow(noisy);
af=fftshift(fft2(noisy));
subplot(2,2,2);
imshow(mat2gray(log(1+abs(af))));
subplot(2,2,3);
imshow(real_img);
af2=fftshift(fft2(real_img));
subplot(2,2,4);
imshow(mat2gray(log(1+abs(af2))));
