mix and match input and output port locations

I've got a subsystem block for which I want an input and an output on the left side, another input and output on the right side, and a third input on the top. Is it possible to move the ports around arbitrarily like this? If it's not possible with a subsystem block, is it possible with an alternative block?

I don't think it is possible. The inports are always on the left and outports on the right. You could rotate the block but always inports on one side and outports on the other side. The enable port or trig port appear on the top side. Most other blocks also appear that way. The round summation block may have inports from top or bottom. But I can't think of any block that can arrange the ports like you wanted.
