i got the retinal image output which is in binary form which has both the vessel and irregular shaped lesions in the image . can i get the output as having only the blood vessel by using bwhitmiss transform with linear structuring element to HIT and disk structuring to MISS

I don't know. Is that one of the published methods? See Section 20.5 of http://iris.usc.edu/Vision-Notes/bibliography/contentsmedical.html#Medical%20Applications,%20CAT,%20MRI,%20Ultrasound,%20Heart%20Models,%20Brain%20Models for how people have successfully done it before. Otherwise, just try it and see.
