How to get transfer function?

I have build a fuzzy logic based adaptive pid control and need to find out a transfer function. How is it possible to do it?

Transfer function of what? Fuzzy Logic Controller does not have a transfer function.
Also, the model looks strange. You basically have 2 PID controllers - one you implement yourself from basic blocks, and then another one you model with PID Controller block.

You can use linmod command it allows to get a continuous-time linear state-space model around operating point
