You can refer to the following File Exchange submission to implement PSO as a Simulink Model :

With VisualEA you can easily create functional models of evolutionary algorithms (EA), its variations, and other similar optimizers from the evolutionary computing family, such as particle swarm optimization and estimation of distribution algorithms.
