how to connect two graph at a plot?

Hello, I have a plot which divided to two region. each region has a graph which are not connected. I am trying to connect these graph however didnot know how? for example if the first graph finish at (1,2), the second one starts at (3,5). I would like to connect (1,2) to (3,5). these graphs are complicated and are results of solving many equations.
