Neural Network GUI PUSHBUTTON LINK

Hi, I want to link my neural network to a push button in order for the user to simulate the the pre-trained generated network. I have five inputs ( bubble, rs, gg, rs, temp). I want these inputs to pass through the network to provide a certain output. Can anyone tell me how to right this code. Please find the attached Picture.
