How to create a lookup table with two inputs and one output?

I have a 3 column table, in which 1st and 2nd column are my inputs and 3rd column is the output required I have to design a lookup table to do the same. How can this be done using a lookup table function in Simulink only.

Have you looked at the lookup table blocks? Documentation here. You are probably looking for the 2-D lookup table. Note that look up tables need the output table data to be of a particular size as defined by documentation here. In your case, it seems like output is a column similar to the inputs. You might need to use functions like meshgrid to get it to work with the look up table blocks. This example could help.
