How can I model Double Acting, Double Rod hydraulic cylinder?

I am trying to model a Double Acting, Double Rod hydraulic cylinder. Is there a Simscape Block that I can use.

Yes. Simscape Fluids (formerly SimHydraulics) has a Double-Acting Hydraulic Cylinder block:

Actually its a double acting single rod cylinder. I have attached some images of cylinder that I am expecting. It should have two "R (Mechanical translational conserving port associated with the cylinder rod)" ports.

You should be able to use the same block... since there's rods on both sides of the piston, all this means is the piston areas are the same on both sides.
As far as "two R ports", you can just as well use the R and C ports provided. If you place other mechanical blocks on either side of the block, it is already given that the motion on the C side is the negative of the motion on the R side. That is, C doesn't have to be "grounded".
