How to get pulse counts of incremental encoder by NI PCIe-6323 and matlab simulink desktop real-time?

I am trying to use a National Instruments PCIe-6323 DAQ with simulink desktop real-time and a quadrature encoder. However, when I run the below model, I get the results as show in scope. My encoder's resolution is 5000r/p and it has A, B and Z. I connect the encoder to NI board correctly according to the below table. So what should I do to fix this problem? Thanks very much.
