Use
im2double
Please dont expect that filtering can remove all noise. You acn try other filters and then do normalization.

If you are looking for contrast improvemnet, have you looked at imcontrast?
Image Enhancement, specially for medical images, you can also look at adapthisteq with proper clipping point.
If you can share the image and details, it help to answer.

Try imadjust(). But global contrast improvement is usually not needed for subsequent processing, like thresholding for example.
