How can I segment this image ?

I am doing image segmentation using Matlab 2019b and I attached the input and the desired output image. I want to segment the input image to have only the lesions same as the desired output image which is attached here.
Any help is appreciated.

Hi,
The above image cannot be solved using the automatic Thresholding and basic image segmentation techniques. Try using Deep Learning if you have sufficient labelled data. If you have unlabelled data, you may use Image Labeler App for generating ground truth data by manual labeling.

Thank you very much for your reply.
Actually I don't have sufficient images to use deep learning. Therefore, I am looking for any other solutions other than deep learning.
