Cannot run MPC adaptive block

Hi,
I'm trying to run my simulink file but I'm receiving the following error:
Error evaluating 'InitFcn' callback of Adaptive MPC block (mask) 'balancerLTVMPC2012b/Adaptive MPC Controller1'.
Callback string is 'ampcblock_InitFcn'
Caused by:
Error using test_balancer (line 49)
Disturbance model "Output" must be an LTI object with 1 outputs.
I've tried exporting the file to a older version but it does not work.
