question about frequency measuring

hello every one , i have a decaying sinusoidal signal in simulink and want measure its frequency using fft and dont know how ? i already did that and connected the fft block after a buffer but there are alot of numeric results of fft and dont know what this results mean and how i can estimate the frequency from them?
