How to change code of machine which works only for predefined data set?

Hello Experts, I have a code for machine for testing and training but it works only for predefined data set. Please help me what should be modified so it would work according to my data set.I have attached the code and data set.Please help me.

I want to know i will need to change my data set or i can change code according to my data set attributes.

i have attached the data set and its first attribute is result that disease occurred or not in 0 and 1 form.
