Linking between Simulink and IBM Rational Rhapsody is not a built-in capability in Simulink. While it is possible - see:

https://www.ibm.com/support/knowledgecenter/SSB2MU_8.3.1/com.ibm.rhp.integ.designtools.doc/topics/rhp_c_int_rhp_and_simulink.html

it is not officially supported, and the technical support for IBM Rational Rhapsody should be contacted for more information.
