Why is the output of my quantized filter in Simulink zero?

I designed a filter using filterDesigner tool. Everything looks correct on paper and in filterDesigner but when I realize both my double precision and quantized models to Simulink, my quantized filter has zero output. What is it that I am doing wrong?

I have attached the Simulink file and the filterDesigner screensots.
